Attention: Important Information on Reporting Injuries
IF THE INJURY IS AN EMERGENCY, DIAL 911 IMMEDIATELY.
- After calling 911, please contact Sondra Kelley toll-free at (866) 761-2884 between the hours of 8:00 AM and 5:00 PM as soon as possible. After hours you may reach Sondra Kelley at (941) 932-5740. This number is available 7 days a week and 24 hours a day.
- Please click on the Howard Leasing Injury Handbook below to fill out your injury paperwork. The First Report of Injury form and witness statements need to be sent to Howard Leasing as soon as possible by fax to (941) 538-7801 or (941) 761-2559 or by email to injuries@howardleasinginc.com
All injuries should be reported within 24 hours, failure to timely report may result in the claim being denied!
All injured employees must submit to a drug screen within 24 hours (*8 hours in Georgia) of when an injury occurs or the claim may be denied.
